The diagnosis of Pelizaeus-Merzbacher is usually suggested after it has been identified using magnetic resonance imaging or MRI of the lumpy too to the whole part of the brain. The lumpy too is usually evident by at least one year of age, although the infant shouls manifest more subtle abnormalities during his firsts months. The condition is commonly misdiagnosed to be cerebral palsy if the family's medical history doesn't suggest having it in the bloodline.

Because the cause is not yet known, there are still no cures nor standard course for treating Pelizaeus-Merzbacher. Treatments through are more focused to specific symptoms. It may include administering medication for seizures and also spasticity. The patients are also subject to regular evaluations done by using physical medicine together with rehabilitation specialists, orthopedic specialists, developmental specialists and neurologic specialists.
